I hope you have well water when you say you "hose your bio falls filter off", because un-chlorinated water will destory any good bacteria....

I have read where concrete is not good for ponds, can leach over time... but then i have read a lot of stuff that isn't true for ponds too, but maybe you should check it out just in case...

Sounds like low oxygen...are you using an air bubbler ? ...maybe test the water for oxygen (expensive meter- but not if your fish are dying)
